Data Protection Privacy Notice

This notice describes how I collect and use personal data. In accordance with the General Data Protection Regulation (GDPR) please read the following carefully to understand our practices regarding your personal data and how I treat it.

To provide you with adequate information on the treatment you are requesting, High Peak Therapy will ask for your personal details so the therapist can assess and contact you.

Information that we collect

​

For the purpose of providing care, when you visit the practice it is on the condition that you give me you're visiting your explicit consent to allow me to document and process your personal data.

​

I will ask for your personal details so I will be able to contact you, the therapist is responsible for all your data.  

​

Personal Details

​

Your name, address, telephone number, email address. The reason for these being kept is to allow High Peak Therapy to respond to your enquires and forward your information on so I can contact you.

​

How I use your information and who I share it with

​

To provide you with adequate information about the therapist and the treatment you require.

​

Providing your personal details to the therapist for them to contact you to book you in for the treatment you require.

​

High Peak Therapy stores your personal record securely locked by password access. I take reasonable and appropriate security measures to protect your personal data.

High Peak Therapy will not share your personal data with anyone without your permission, unless there are requirements to do so under any legal or medical obligation.

​

I will only disclose your information on a need-to-know basis and will limit any information that I share to the minimum necessary.

​

Keeping your information safe

​

I store your personal information securely on our computer system and your information cannot be accessed by those who do not work for High Peak Therapy.

​

I take precautions to ensure security of my office and filing systems and I use password protected systems to record and use your personal information safely and effectively. 

​

I keep your records for 7 years, as per our regulatory requirements.

​

Access to your information and other rights

​

You have a right to access the information that I hold about you and to receive a copy. You should submit your request in writing to High Peak Therapy, I have 30 days to respond to your request. Any access request may be subject to a fee of £10 to meet my costs in providing you with details of the information I hold about you.

​

 You can also request us to: 

  • Correct any information that you believe is inaccurate or incomplete. If I have disclosed that information in a capacity to other medical professionals, I will let them know about the change

  • Erase information I hold although you should be aware that, for legal reasons, I may be unable to erase some information due to Regulatory Requirements

  • Supply your information electronically to another medical professional
